All degree seeking international students, degree seeking Graduate Assistants and Fellows are required to purchase this plan or show proof of comparable coverage.

Credit hour requirement can be met by a combination of online and on-campus credit hours, not to exceed 50% online.

Eligible students may also enroll their dependents.

Students enrolled in the student health insurance plan at registration and charged to their student account.

Enrollment must be within 31 days of the qualifying event. Newborn enrollment must be within 60 days after birth. You may be requested to provide documentation verifying the date of the qualifying event (e.g. birth certificate, marriage license, I-94 travel history, boarding pass, letter showing a loss of coverage). Coverage will be effective beginning the first day following your loss of coverage.
